Should an accident or incident occur at the work site, it is essential to investigate the incident so that the employer can document exactly by all means practical, what happened, who was injured, what safety practices were being used, and to make necessary changes in the safety program.

3. Investigations are thoroughly researched, photographs are taken, witnesses are interviewed, and statements are taken.
